Emergency Medical Authorization Form
This form is required by our district for the care of your child in case of emergency. A new form should be filled out at the beginning of each school year and if information changes throughout the year. Page 1 of this form states the Ohio Revised Code.

Student Medication Request Form
This form is required for the school to administer prescription and non-prescription medications. Please complete form & return to building with medication in original container.

AUP ~ Acceptable Use Policy for Student
This is our policy for parents giving permission for computer use.
